Multifaceted Social Service agency serving elderly and persons with disabilities, seeks Home Care Case Manager-hybrid schedule to assess consumer needs, conduct home visits, develop service plans, and maintain case records. Bilingual applicants encouraged to apply.

Home Care Case Manager - Social Services - hybrid schedule
Job requirements

Bachelor's degree preferred in a related social service field. Bilingual skills and/or 3 year of experience in a related field may be substitured for education. Case management or experience with elders preferred. Must possess computer skills. Use of a reliable vechile including active vehicle insurance with a current license to drive required.

A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year. The bonus for this position is $1,000.00.
