Job Description:
Licensed Clinical Social Worker (LCSW) or Master of Social Work
(MSW)We are seeking a Massachusetts Licensed Clinical Social Worker
(LCSW) or Master of Social Work (MSW) for our Facility in Salem,
MA. Must have experience working in a Skilled Nursing Facility and
be familiar with working with the post-acute care patient
population. The LCSW or MSW will utilize individual and group
therapy skills to assist patients and their families in achieving
and maintaining a maximum level of physical, social and emotional
functioning via teleconferencing. Responsibilities include working
as part of a multi-disciplinary treatment team to advise family
caregivers, and patients, and help plan a smooth transition to the
next level of care.LCSW or MSW consistently demonstrates support of

to qualify for this position ***Principal Responsibilities:1.
Interviews patients and family members to obtain information about
home, social environment, family relationships, health history,
patient's functioning and treatment goals2. Explains patient's
background, needs, and family dynamics to the team and participates
in the planning of a comprehensive post-hospital care program for
the patient3. Collaborates with Licensed Clinical Social Worker to
determine the most effective social work treatment modality and
carries out therapy to facilitate the adjustments to disability and
resolve psychosocial problems with patient and/or family4.
Collaborates with other health care professionals to develop,
manage, and measure the effectiveness of the transition program5.
Assists patient and family with community integration and referrals
including DORS/DOA/Private Attendant Care Services, Home Health
referrals, Continuum referrals, School reintegration, and
Outpatient Services6. Identifies eligibility and assists family in
applying for entitlements. Investigates alternative sources of
funding for uncovered services in the community7. Participates in
professional organizations and activities; assists in the
orientation of other health care professionals in understanding the
services and purpose of social work8. Perform all other duties that
may be assigned in the best interest of the hospitalKnowledge,
Skills & Abilities Required1. Master's Degree in Social Work from
an accredited university required2. Current Massachusetts
designation as a LCSW or MSW required3. Demonstrated understanding
of the principles, practices, techniques and professional standards
in the field of social work4. Knowledge of community resources,
health care system and interdisciplinary functioning5.
Interpersonal communication, advocacy and analytic skills required
to work with patients and families, and various medical
