SalaryUSD $27.00/Hr.Description and ResponsibilitiesThe Licensed
Practical Nurse works in close collaboration with the Registered
Nurse to ensure the medical needs of people supported are met. This
position involves acting as a liaison between the program and other
stakeholders including residential program, family members, and
other medical providers. The LPN can expect to provide support to
people served while at the program and while out in the community.
This is a 40-hour position which will be assignedto residential
programs in both Grafton and Baldwinville. Work in close
collaboration with the Registered Nurse to maintain medical records
and oversee proper documentation. Administer and monitor
individuals' medical needs in accordance with orders by prescribing
physicians as needed. Maintain ongoing relationship with other
health care providers, to provide consistent carry over of
prescribed treatment procedures techniques, etc. Assist with staff
training on current medical issues, procedures and techniques,
including MAP training. Maintain relationship with residence as to
current status and to obtain medications. Provide coverage as
needed. Participate in the program Health and Safety Committee.
Attend in-service trainings as related to general job
responsibilities. Advocate for individuals served with physicians
and other service providers. Ensure physical plant maintenance,
included but not limited to health & safety and cleanliness of the
program. Acquire and maintain all required certifications. Perform
other related tasks as assigned.Qualifications Must be
Massachusetts licensed. Minimum 2 years experience working with
people with developmental disabilities and behavioral involvement
preferred. Physical stamina and lifting individuals served is
