POSITION TITLE: Licensed Practical Nurse (LPN) FLSA STATUS: Non-Exempt REPORTS TO: Nurse Manager SUPERVISES: N/A DEPARTMENT: Home Care COMPANY: OVNA **$3k sign-on bonus eligible for FTE's.. POSITION SUMMARY: The LPN provides individualized nursing care and comprehensive services to patients and families in their own homes under the supervision of a Registered Nurse. Provides basic patient care under the supervision of a Registered Nurse in accordance with physician's orders
Observes patients, charting and reporting changes in patient's conditions, such as adverse reactions to medications or treatment and taking any necessary action
Educates patient and families in self-care and health management
Attends case conference as requested
Adheres to policies and procedures
Participates in agency quality management and safety activities QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Current license to practice as a practical nurse in the state of Massachusetts
Current CPR required
Two years nursing experience
Current MA driver's license and reliable transportation Knowledge, Skills and Abilities: Language Ability: Ability to read, write, speak and understand the English language. Knowledge of the structure and content of the English language including the meaning of words, rules of composition, and grammar. Ability to effectively communicate with peers, supervisor, residents, clients, and guests.
Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measurement, using whole numbers, common fractions and decimals.
Cognitive Demands: Ability to apply common sense understanding to carry out instructions provided in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Able to swiftly change direction as necessary.
Computer Skills: Ability to use Microsoft Windows, email and the Internet. Ability to use software/device for data entry of visit documentation.
Competencies: As outlined in Orientation plan. PHYSICAL REQUIREMENTS/WORKING CONDITIONS: Works primarily indoors in varying climate controlled settings
Possible exposure to unpleasant odors
Possible exposure to chemicals as identified in the MSDS Manual
Subject to hostile and emotionally upset family members, personnel and visitors
May work beyond normal working hours and on weekends and holidays when necessary
