SalaryUSD $33.00/Hr.Description and ResponsibilitiesOpen Sky's Shared Living programs allow adults with disabilities to become part of a host family, with all of the benefits and responsibilities that sharing a life and a household entail. The host family provides emotional support as well as role-modeling, guidance on community engagement, and activities of daily living. The host family also provides basic care for things like medication management, hygiene, funds management, and ensuring transportation is provided to appointments. -Shared Living teams consist of visiting nurses and service coordinators, who provide expert assistance when necessary. Individuals will have goals of independent living, stepping down into lower level of care, or permanently requiring support and supervision. -Registered Nurses visit families in their homes for a caseload of placements in the Central Massachusetts region, operating out of an office in Whitinsville. -This is a full time position and is predominantly first shift Monday-Friday. There is no on-call responsibility. -Key Responsibilities Include:

• Completion of intake and annual assessments.
• Maintain necessary contact with medical service providers.
• Hands-on requirements limited to taking vitals and blood pressure readings, and general overall visual assessment.
• Upkeep of typical documentation and progress notes in electronic healthcare record (eHana).
• Provide medical teaching and training to caregivers and persons served, and peers/teammates as needed.
• Perform medication audits as needed.Qualifications
  • Must be a currently licensed RN in Massachusetts.
  • Motivated with ability to work independently and autonomously.
  • Valid driver's license, acceptable driving record, and reliable transportation.
