Job Description:

ACE Medical (a member of the Sevita family) is hiring a Full Time Physical Therapist Assistant for our Home Care department. A home care physical therapist assistant works under the supervision of a physical therapists who helps treating patients that are suffering from physical disability in a home care environment. These physical disabilities are often due to injury, disease or surgery. Their typical duties and responsibilities include:

• Provides direct physical therapy treatment to patients according to instruction of Physical Therapist, and in accordance with the written plan of care.
• Observes patient and reports to Physical Therapist and department personnel the patients reaction to treatment and any changes in patients condition.
• Instructs patients, family and/or caregiver in proper techniques to assist patient in achieving maximum level of independence.
• Maintains necessary records, progress notes, and conference notes, which are incorporated into the patients clinical record within established time frame
• Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the expectations of Sevita Health standards of care guidelines. Qualifications:
• High School Diploma or equivalent.
• One year of experience in a human services or related field.
• Current PTA license in the state of work.
• Good working knowledge of physical therapy equipment and therapeutic exercise treatments.
• Self-motivated and detail-oriented with ability to multi-task.
• Reliable, responsible, and caring nature.
