Job Description:
Summary:The Community Health Worker/Patient Navigator (CHW) will
work to outreach, identify, recruit, and support patients to engage
in care and/or care management services. The CHW will provide
patient education and advocacy to assist patients with accessing
services of FHCW and as appropriate, make and follow-up on
referrals for external services. The CHW will support the care team
with patient follow-up, reminders, and will address barriers to
care. The CHW will document all services as required and will
assist in meeting all program requirements. - - -Educational
Requirements: -High school diploma or GED required; Bachelor's
degree in a related discipline preferred. Preferred
candidate will have Community Health Worker or Medical Interpreter
certificate of completion. - - -Experience Required: -Minimum of
two years relevant work or college-level internship experience
required. Data entry skills and proficiency with MS Office
including Excel and Outlook. Proven ability required to perform
patient interviews, coordinate and collaborate with internal
services and external community partners. Must have
demonstrated
organizational skills and the ability to work independently. - -
-Professional Licensure/Special Skills and Certification Required:
-Must be bilingual in English and another language appropriate to
the health center's patient population. Local travel required for
home visits to
patients. Community Health Worker Core Training required within
first year of employment. Flexible evening work, weekend hours
required to meet FHCW community or patient needs. - -Interpersonal
Relationships/Organizational Expectations: -1. Commitment to
providing health care to families in a medically underserved
community. -2. Willingness to be flexible in the development of new
methods of health service delivery. -3. Ability to deal with
sensitive information in a highly confidential manner. -4.
Effective communication skills, both oral and written, with
patients, families, visitors, co-workers, health care
professionals, funders and other regulatory agencies. -5. Ability
to function independently with minimal supervision, and to plan,
coordinate, and carry out those tasks necessary to meet the job
responsibilities. -6. Awareness of resources available within the
health center to accommodate the needs of patients. -7. Commitment
to providing cost conscious, fiscally responsible, timely,
efficient, effective, safe, patient-centered, culturally competent,
equitable, and participatory health care. -8. Processes personal
health information in accordance with FHCW HIPPA policies and
