Job Description:

Family Practice-Without OB opening in Worcester, Massachusetts. Community Health Worker (CHW) Bilingual (English/Spanish or Portuguese) Community Health Workers / Health Educators are needed for local health center in Worcester, MA.

• Candidates will be trained to facilitate evidence-based education, such as:
• CDSMP - Chronic disease Self Management Program
• DSMP- Diabetes Self Management Program
• MOB- Matter of Balance
• DPP-Diabetes Prevention Program Prior experience facilitating health education or ESL - English as a second language classes is recommended. She/he will be able to deliver information to diverse audiences and communities. Individuals interested should have high communication skills (oral and written), and able to reach multilingual audiences in English, Spanish, or Portuguese, as participants speak mostly those languages. She/he will work with Patient Navigators to inform clients and patients of up coming activities and events in the community. He/she will educate participants about health, ESL, diverse health education modules and to inform them about the benefits of the COVID-19 vaccines, social distancing and other public health recommendations during the pandemic. Health Educators-CHWs will be responsible for educating and recruiting participants via zoom or in person to offer them appropriate programs, COVID-19 vaccine services. Requirements:
  • Bachelor's degree required
  • Experience in human or health services
  • Excellent written and verbal communication skills necessary
  • Car required Spanish or Portuguese or Chinese (Mandarin) speaking
    Salary commensurable with experience, full time and part-time positions with benefits are available. M - F 35 hours / week $20 per hour
