POSITION SUMMARY:The primary purpose of the position is to conduct
and coordinate the development and completion and submittal of an
initial MMQ for each new MassHealth member at the end of 30 days
from the admission date, or at the end of 30 days from the
conversion date from private or Medicare coverage to MassHealth
coverage. An MMQ must also be submitted semiannually for all
MassHealth members who are residents of a nursing facility.This
position is a Part-Time position. Prospective candidates available
days and schedule are flexible for this position.THE OVERLOOK

Administrative Functions:
For new members, codes must reflect the care provided on the
effective date forward for 30 days. For established residents,
codes must reflect the care provided during the previous month. A
temporary condition may not be claimed. A temporary condition is
one that requires service for less than 50 percent of the month.
All services claimed must be medically necessary.
The medical record is the source for information to complete the
MMQ. Documentation must be complete, accurate, dated, and signed by
the person performing the care. The licensed nursing summary, daily
licensed nursing notes, physician's orders and progress notes, ADL
flow sheets, medication administration records, treatment records,
and care plans should all be reviewed to complete the MMQ.
Documentation for assistance with activities of daily living must
be associated with resident dysfunction, and the reason given for
assistance must relate to this dysfunction as described in the
medical plan.
The following terms should not be used in documentation, as they
are not specific: frequently, almost always, often mostly
dependent, and almost total assist.
If a member has been in the facility for less than a month, the
score is based on 50 percent of the days the resident has been in
the facility.
Initial MMQs and semiannual MMQs must be signed by a registered
nurse. Clinical records must document this activity before the
information is forwarded to the nursing facility staff who are
responsible for preparing the electronic submission of MMQ data.
When Medicare coverage ends, the member is eligible for conversion
to MassHealth. The facility must submit an MMQ (Reason Code 2 for
conversion) with an effective date of the first day of MassHealth
eligibility. Submit the MMQ at the end of the month following the
issuance of the member's 12-digit member identification number.
Completing the Semiannual MMQ
The semiannual MMQ must be submitted for every MassHealth member
who is a resident of the facility on the first day of the reporting
period.
The semiannual MMQ must be completed from documentation for the
previous month. It is essential to obtain semiannual MMQ
information at the same time for each reporting period.
With each semiannual MMQ, indicate the discharge of a resident who
is no longer a MassHealth member as of the first day of the current
reporting period. For example, if the effective date of the
semiannual MMQ period is July 1, 2009, do not enter a discharge
that occurred on July 3, 2009.
Committee Functions:
Work with the Interdisciplinary Care Plan Team in developing a
comprehensive resident assessment and care plan for each resident.
Serve on, participate in, and attend various other committees of
the home as required, and as appointed by the DNS or her/his
designee.
Provide written and/or oral reports of the care plan functions as
requested or directed by committees.
Participate in functions involving discharge plans as may be
necessary.
Personnel Functions:
Maintain an effective, friendly working relationship with health
professionals, physicians, consultants, and governmental agencies
that may be involved in the resident assessment/care plan functions
of the facility.
Meet with and solicit advice from department supervisors concerning
the resident assessment/care plan functions of the home.
Nursing Functions:
Evaluate each resident's pertinent medical data to determine if
codes reflect the care provided on appropriate dates/within
appropriate timeline and are medically necessary.
Audit the resident medical record for information required to
complete the MMQ. Audit should confirm where documentation is
complete, accurate, dated, and signed by the person performing the
care. Audit to include: the licensed nursing summary, daily
licensed nursing notes, physician's orders and progress notes, ADL
flow sheets, medication administration records, treatment records,
and care plans.
Evaluate each resident's medical record for documentation of
assistance with activities of daily living associated with resident
dysfunction, and to ensure the reason given for assistance must
relate to this dysfunction as described in the medical care
plan.
Ensure that care provided is in accordance with the resident's
wishes.
Act as a member of the nursing on-call rotation.
Support the clinical team with hands' on support as required in
this position.
Staff Development
Attend and participate in all annual OSHA, CDC, and DPH required
in-service training programs.
Attend and participate in continuing education programs designed to
help you keep abreast of changes in your profession, as well as to
maintain your license on a current status.
Care Plan and Assessment Functions:
Ensure that appropriate health professionals are involved in the
assessment.
Ensure all members of the assessment team are aware of the
importance of completeness and accuracy in their assessment
functions and that they are aware of the penalties, including civil
money penalties, for false certification.
Assist the Unit Manager, Nursing Supervisor and relevant
supervisors of other departments in ensuring that all personnel
involved in providing care to the resident are aware of the
resident's care plan and that nursing personnel refer to the
resident's care plan prior to administering daily care to the
resident.
Ensure quarterly and annual resident assessments and care plan
reviews are made on a timely basis.
Assist the Unit Manager, DNS and other relevant directors and
supervisors in maintaining good rapport and morale between and
within departments to ensure that a team effort is achieved in
developing and implementing a comprehensive assessment and plan of
care.
Resident Rights:
Maintain the confidentiality of all resident care information.
Ensure that all residents are treated fairly, and with kindness,
dignity, and respect.
Ensure that resident interviews are conducted in private.
Ensure that all members of the assessment team and the
interdisciplinary care planning/medical and nursing team are
knowledgeable of the residents' responsibilities and rights
including the right to refuse treatment when formulating the care
plan.
Ensure that the facility's policies and procedures governing
advance directives are reviewed with the resident and/or
representative.
Report all allegations of resident abuse and/or misappropriation of
resident property immediately, follow the home's policy.
Review any complaints or grievances made by residents regarding the
resident assessment or care plan.
Ensure that a written record is made of any resident's complaints
or grievances with a full accounting of any actions taken and the
current status of the complaint, report to DON.
Ensure that resident's rights are well established and maintained
at all times.QUALIFICATIONS:
Must possess a working knowledge of MMQ and Care Planning Process.
Must be able to read, write, speak, and understand the English
language.
Must possess the ability to make independent decisions when
circumstances warrant such action.
Must possess the ability to deal tactfully with personnel,
residents, family members, visitors, government agencies/personnel,
and the general public.
Must be knowledgeable of nursing and medical practices and
procedures, as well as laws, regulations, and guidelines that
pertain to long-term care.
Must possess leadership ability and the willingness to work
harmoniously with other personnel.
Must possess the ability to plan, organize, develop, implement, and
interpret the programs, goals, objectives, policies and procedures
that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition and enthusiasm, as
well as the willingness to handle difficult residents.
Must be willing to seek out new methods and principles and be
willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resident's
condition.
PHYSICAL REQUIREMENTS/WORKING CONDITIONS:
Moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with residents, personnel, visitors, government
agencies/personnel, under all conditions and circumstances.
Is subject to hostile and emotionally upset residents, family
members, personnel, and visitors.
Communicates with the nursing personnel, and other department
supervisors.
Works beyond normal working hours and on weekends and holidays when
necessary.
Is subject to call-back during emergency conditions and
participates in a rotating on-call schedule.
Is subject to injury from falls, burns from equipment, odors, etc.,
throughout the work day, as well as to reactions from dust,
disinfectants, and other air contaminants.
Is subject to exposure to infectious waste, diseases, conditions,
etc., including TB and the AIDS and Hepatitis B viruses.
Maintains a liaison with the residents, their families, support
departments, to adequately plan for the residents' needs.
Must not pose a direct threat to the health or safety of other
