Supplemental Health Care is seeking a Nurse Practitioner Family
Practice for a travel job in Worcester, Massachusetts.Job
Description & Requirements
Home Health Nurse Practitioner Contract Details:- $2,900 - $3,100
per week- SHC Home Health Nurses are paid by the hour, not by the
visit- Travel and mileage reimbursement available on all contracts-
Nurse will have a designated territory of coverage- Weekly
expectation is 30 units of productivity- Home Health contracts with
SHC require previous experience with OASIS/Medicare Certified
Visits- Must be a Road warrior, Tech savvy, flexible with change,
teamwork, building a relationship with patients- Travel within 60
mile Radius of territory in any direction- No on call for
overnight/weekends- Must be flexible with scheduling.- 3-7 patients
per day in home- No hard "end time" for working day- Will be
completing all visits in the patient's home (or other specific
location agreed upon)- Heavy patient education- Helping to manage
comorbid conditions (hypertension, diabetes, etc.)- Heavy
documentation - no additional admin. time built into the schedule
for NPs.
Types of visits:- Treatment visits (Diagnose/treat)- Post Hospital
Discharge (Urgent visits)
-Annual wellness assessments (1 time/year) diagnose and checkup to
properly document risk score.- EMRs (Electronic Medical Record) -
Athena is the primary EMR- Salesforce as a secondary EMR with
patient info that you may not find in Athena.- Mileage/travel
reimbursement- Must use Concur GPS app and own vehicle- Reimbursed
at $0.44/mile- Use personal phone - must have unlimited
data/hotspot- Reimbursed $65/month for cell phone- Ring Central -
app with business line assigned- Expect patient calls from 8 am-10
pm in your time zone.

Requirements:- OASIS/Medicare Certified Visits Experience-
Electronic Charting experience- 2+ years' experience as a NP
required (AL, FL, MD, VA, CT, DDE- Nephrology not required -
hospice, in-home care, palliative care translate well- No
Addiction, Pain Management, or Botox as NP experience per clinical
team.- Value based care highly preferred- Most contracts require
some on-call- Current Massachusetts Nurse Practitioner License-
American Heart Association BLS- Valid U.S. Driver's
