of the State University of New York invites applications for the
position of Nurse Practitioner or Physician Assistant in the
division of Endocrinology. The Nurse Practitioner or Physician
Assistant will see and treat patients with diabetes admitted to
Stony Brook University Hospital and will demonstrate skills in
collaboration, teamwork and problem solving to achieve desired
goals. Independently assess and evaluate new and follow-up consults
to the inpatient diabetes consultation service at Stony Brook
University Hospital. Perform daily physical exams of patients with
diabetes with special attention to body systems that may be
affected by diabetes. Review 24-hour tests of blood glucose values
and recommend daily orders for medication to primary clinical team.
Educate patients and family members regarding diabetes
self-management principles based on recognized curricula and
national standards of care. Identify discharge needs at the point
of initial patient contact and assist in the transition of care
planning. Rotate through the established \"call \"schedule of the
diabetes service and carry a paging device. Demonstrate skills in
collaboration, teamwork and problem solving to achieve goals.
Knowledge of basic software packages and hospital information
systems (EMR). SPECIAL NOTES: This position requires 8-hour shifts
and includes rotational coverage of weekends and holidays.
Qualifications REQUIRED QUALIFICATIONS: Nurse Practitioner: NYS
license and certification as a Nurse Practitioner. Master's degree
(foreign equivalent or higher) in nursing or completion of a course
for a Nurse Practitioner. Physician Assistant: NYS license and
certification as a Physician Assistant. Bachelor's degree (foreign
equivalent or higher) from an accredited school of Physician
Assistants. Must be eligible to sit for and pass the exam for
Certified Diabetes Care and Education Specialist (CDCES) within 1
year of start date. PREFERRED QUALIFICATIONS: Certified Diabetes
Care and Education Specialist (CDCES). Three to five years acute
care as a NP/PA experience in diabetes management and education in
a hospital setting. Knowledge of current clinical guidelines and
standards from the American Diabetes Association, American
Association of Clinical Endocrinologists and American Association
