Clinical Success Mentor, you’ll contribute to our success in the
following ways: - Act as a liaison and maintain communication with
new hire and current clinicians, branch leadership, and preceptors
to provide practical clinical experience and guidance to field
clinicians, including standardized orientation of new clinicians. -
Ensure execution of the new hire field clinician orientation
experience via: - Preceptor Guide - Competency Checklist - State
specific requirements as needed - Demonstrate processes for SOC,
ROC, Recert and routine visits to clinicians during orientation
process - Work with preceptors and/or clinicians to review
application of clinical documentation that meets regulatory and
compliance standards and guidelines - Provide education to ensure
effective collaboration with healthcare providers in accordance
with physician’s orders and plan of care. - Continually work to
create a culture of learning and support for preceptors and new
clinicians as they are gaining confidence and competence in their
respective role. - Meets with Preceptors and/or Orientees on
regular basis to assess progress during Orientation. Communicates
with CTMs and/or BDs as needed to discuss progress of Orientees. -
Collaborate with preceptors and local leadership team to determine
& participate in the delivery of targeted education based on needs
and process improvements to align with Elara goals and vision -
Facilitate annual skills lab & competency assessments as needed -
Other duties as assigned to support the changing needs of the
business. What is Required? - Diploma, Associate or Baccalaureate
Degree in nursing - 1 year home health experience - Previous nurse
preceptor or educator experience is preferred - Must have a
current, unrestricted RN license in the state you are working, may
be required to obtain additional state licensure. - Proficiency
with HomeCare HomeBase & PointCare is preferred. - Demonstrates
strong leadership, effective communication and interpersonal
skills. - Ability to travel 50% travel required (mainly to branches
in your region) - Proficiency with OASIS You will report to the
Director of Education. The base salary for this position is $85,000
to $95,000 annually , based on the company’s good faith estimate at
