Job Description Job Description St. Anne Institute Job Description I. TITLE: Per Diem Therapist – Community Based Clinical and Care Management Services II. SUPERVISOR: Program Administrator - Community Based Clinical and Care Management Services - Hudson Falls III. HOURS PER WEEK: As needed (includes some early evening hours) IV. QUALIFICATIONS: LMSW/LCSW, LMHC or other relevant degree and experience working with individuals, families and groups preferred. Excellent communication and writing skills required. V. FUNCTIONS: To provide individual, family therapy and group therapy to assigned clients. VI. RESPONSIBILITIES: Coordinate the clinical treatment plan and develop comprehensive clinical assessments and treatment plans for assigned individuals and families. Provide individual, group and family focused treatment/therapy including outreach and advocacy. Coordinate and complete necessary reports and correspondence Work cooperatively with client’s case manager and/or outside service providers to coordinate clinical services and resolve issues. Maintain updated assessments and progress notes, and record updates in the SAI Electronic Health Record. Keep abreast of current trends in clinical treatment, trauma-informed care and evidence-based models through reading, in-service training and conferences. Adhere to NASW, AMHCA, NYS Justice Center and SAI Codes of Conduct/Ethics, and ATSA Guidelines.
