Registered Nurse (RN) | Home Health Location: Bennington, VT Agency: Cynet Health Pay: $2,578 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/2/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Bennington, Vermont, 05201! Job Title: Home Health RN, Hospice RN Profession: Nursing Specialty: Home Health RN, Hospice RN Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8am-4pm with some weekends and on-call required Experience: Minimum 2 years License: Valid state license as a registered nurse Certifications: CPR certification required Must-Have: - Strong understanding of the nursing process - Experience with OASIS documentation - High-Tech skill competency in IV use and maintenance, wound VAC, complex wound care Description: Occasional home care visits are required. Utilize the nursing process to assess patient and caregiver needs to provide quality care to assigned patients. Develop and communicate a realistic plan of care for each patient. Ensure that patient and caregivers are included in the care plan process. Serve as case manager for assigned patients and actively manage caseloads. Effectively complete OASIS documentation in accordance with agency policies and regulations. Implement medical plan of care, nursing interventions, and procedures to ensure safe, effective quality of care. Stay up-to-date on various High-Tech skills, including IV use and maintenance. Ensure that infection control standards are maintained throughout the plan of care. Collaborate with LPNs to ensure that the plan of care is followed and patient needs are met. Consistently review documentation, annotate visit notes, and act on recommendations. Evaluate various aspects of the implemented plan of care to meet patient and caregiver needs. Maintain up-to-date and accurate documentation of nursing care provided. Follow agency protocol with processes related to documentation and patient management. Provide education to patients and caregivers to ensure informed decision-making. Identify interdisciplinary needs of the patient and document accordingly. Fill out a variety of agency, state, and federal forms accurately and in a timely manner. Carry out visits in a time-effective manner and communicate scheduling needs promptly. Assess patients' needs for healthcare services and develop and coordinate plans of care. Transmit patient incident reports to quality improvement specialists in a timely manner. Coordinate safe patient discharges and/or transfers and ensure appropriate processing of paperwork. Facilitate communication with physicians, providers, and team members to support the recommended plan of care. Participate in monthly peer review audits and provide constructive feedback. Schedule, coordinate, and facilitate case conferences to enhance communication among team members. Advocate for patient care issues to ensure that care is tailored to each patient's and family's needs. Engage in interdisciplinary team meetings to develop individualized plans of care. Stay informed about pain control and symptom management strategies. Ensure effective education for patients and caregivers related to treatment options and instructions.
