ER Experience: 2 years Licenses: RN-MA(Copy needed) Certifications:
ACLS(Copy Needed); BLS(Copy Needed); CERT-TNCC(Copy Needed) Work
History: Should not be more than 90 days of gap in entire work
history References: 2 References in entire work history Include
past 7 years of work experience (If less than 7 years of Nursing
experience, non-clinical work history must be provided to fulfill
7-year WH requirement here). 2 recent references, minimum 1
Supervisor or Above. References must be from same skillset of the
order being submitted to. RN License Copy On-File Education must
include city and state Can Pass Fit Test: Y/N Answered Flu Vax: On
File or Willing to Obtain Answered (Declinations Not Accepted)
