LanceSoft is seeking a travel nurse RN Home Health for a travel nursing job in Rutland, Vermont. Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 03/30/2026 - Duration: 13 weeks - 37 hours per week - Shift: 8 hours, days - Employment Type: Travel OASIS experience required. 1. Utilizing the nursing process, assesses patient/care giver needs in order to provide quality care to assigned patients, in accordance with evidence-based practice, State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations 2. Develops and communicates a realistic plan of care for each patient in accordance with evidence-based practice, State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations. Ensures that the patient/care givers are included in the care plan process. 3. Serves as Case Manager for assigned patients and actively manages case loads and visit times in accordance with agency productivity standards. 4. Effectively fills out and completes OASIS documentation in accordance with Agency policies and procedures, and State and Federal regulations, Demonstrates knowledge of, and keeps up-to-date on changes to OASIS documentation requirements as needed. Ensures documentation is competed in accordance with Agency practice (timeliness.)
