Physical Therapist Location: Southborough, MA Agency: GQR Healthcare Pay: $2,785 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Home Health Physical Therapist Location: Southborough, MA Job Description: We are seeking a Home Health Physical Therapist for a 13•week contract, Monday to Friday, 8:00 AM to 4:30 PM. Provide quality care to patients in their homes, manage cases, and collaborate with healthcare professionals. Key Responsibilities: • Deliver physical therapy services to home health patients. • Complete documentation after each visit. • Manage daily cases, including Start of Care and at least four visits. • Collaborate with healthcare team for comprehensive care. Requirements: • Valid MA Physical Therapist License. • Basic Life Support (BLS) certification. • 2 years of home health experience. • Familiarity with EMR, especially Home Care Home Base, preferred. Additional Information: • No on•call or holiday duties. • Possible floating to other locations if needed. • COVID•19 vaccination not required.
