Located off the shores of Lake Quinsigamond within the woods of the Worcester Land Trust, our Human Resources Department seeks an experienced full time Payroll & Benefits Coordinator to join our team.Responsibilities:Under the guidance and supervision of the Director of Social Services and working in collaboration with the Nursing and Spiritual Care Departments, the incumbent provides social services as needed to the residents of NDHCs long term and skilled nursing care residents. Responsibilities include, but are not limited to, the following: Completes clinical assessments and social service documentation as required by state / federal mandates and facility policy,Interviews residents and their family members to solicit information regarding the residents strengths, weaknesses, coping strategies and lifestyle prior to placement and develops and initial psychosocial assessment along with a detailed social history within the clinical record,Develops and maintain resident psychosocial assessments and quarterly/routine care plans,Educates residents, family members, and / or significant others regarding advance care planning and community resources,Works with the interdisciplinary team to assure resident and family concerns are addressed in a timely manner. Advocates for the needs of residents in ensuring resident rights,Educate residents, families, and staff on resident rights, resident abuse, mistreatment and neglect regulations to ensure residents are protected,Provides information and documentation to residents and families with regards to access to Medicare, MassHealth, and financial and insurance benefits. Collaborates with social service team members to ensure the needs of residents are met with professionalism, promote resident dignity, and resident choice.PM17.Requirements:Licensed Social Worker (LSW or LSWA) required. Masters degree in Social Work required. Minimum two years experience working in the human services and/or geriatric services field required.
