Adult Community Clinical Services(ACCS) are comprehensive clinically focused services provided by a multi-disciplinary team that provides services to adults with mental health needs. The Bridge is currently seeking qualified candidates in the areas listed below:
10 Licensed Clinical Staff
Complete, in collaboration with the training department, all Screening and Enrollment of Persons served referred to ACCS in compliance with DMH. Identify engagment strategies, considering the person's situation and preferences, which can be utilized by the team. Screen, assess and reassess Person for risk and protective factors using agency risk assessment protocols and tools. Oversee the development and implementation of Safety Plans and Crisis Prevention and Response Plans and develop appropriate critical need interventions and transition plans. Build relationships with Persons and develop shared understanding of Persons' goals and the steps necessary to achieve the goals. Ensure timely completion, with Persons served and other members of the Integrated Team, the Comprehensive Assessment and other clinical assessments as needed. Participate in the development and implementation of Treatment Plans and Treatment Plan reviews. Provide direct intervention with the Person served in line with the Person's Treatment Plan. Develop and facilitate clinical and support groups based on the needs and preferences of Persons served. Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with people served and family members. Demonstrate respect for all Persons regardless of ability status, culture, sexual orientation, gender identity, religion, or ethnicity. Monitor person's needs and progress; design and deliver rehabilitative interventions to assist person in building and using skills. Provide clinical consultation to programs, staff teams, and Persons served on an ongoing basis; respond to serious events as needed. Provide modeling, supervision, and training to direct care staff, especially related to evidence-based interventions. Develop, coordinate, and deliver trainings in conjunction with the Integrated Team members, as needed to address the clinical learning needs of staff (e.g. orientation, crisis intervention, developing a clinical perspective, etc.). Collaborate with applicable CAre Coordination Entities, Emergency Service Providers, outpatient clinics and other providers/entities to share information and ensure that the needs of the person are fully met. Develop productive affiliations and partnerships with other community-based providers to enhance the agency's service delivery network. Provide support and assistance to persons through Care Transitions and other life changes, making such changes as seamless as possible. Assist Program Leaders to identify and evaluate referred Persons' risk behaviors and develop appropriate transition plans and interventions. Coordinate risk management data; assist in the development and implementation of corrective action plans in conjunction with other members of the Integrated Team. Support and monitor the implementation of identified clinical approaches and evidence based practices ascribed to by the agency. Participate in On-Call rotation and critical incident review process. Obtain and maintain MAP certification Attend, participate in, or facilitate meetings, trainings, and supervision as assigned. Perform other duties as assigned.
Master's Degree in social services field. Must be a Licensed Practitioner of the Healing Arts(LICSW, LCSW, LMHC, LRC, LADC I, Physician, RN, OTR/L, or pyschologist)as well as two years of professional experience working. Valid driver's license required.
