  • Put Members FirstAbout our Team:We employ a field-based, home-based care model and are committed to meeting members where they are-in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.About the Role:In this role, you will be the central coordinator of Cityblock's LTSS efforts, in collaboration with internal care teams and external LTSS Coordinators, ASAPs and ILCs.Your responsibilities cover three major areas across the LTSS domain:
    • Administration:
      • Populating, submitting and tracking LTSS requests. This will involve direct coordination with Cityblock's RN care management team
      • Submitting initial referrals for assignment of an LTSC to ASAPs/ILCs
      • Coordinating the payer UM RFMI (request for more information) process
      • Tracking assessments/reassessment submissions and following up with LTSCs if not sent within designated timelines
      • Uploading LTSC assessments and visit notes into Cityblock's care management system (Commons)
      • Coordinate PCA re-authorization timing with MDS assessment timing with the goal of performing the MDS and skills assessment at same time
      • Relationship Building: You're Cityblock's primary, day-to-day connection to LTSS provider organizations, skills trainers, ASAPs & ILCs. You'll represent Cityblock in regular meetings with their management and front-line teams, and support them with daily services as needed.
      • Reporting: You'll maintain regular reporting to external parties (ASAPs/ILCs), and to track performance and service levels internally.You may also have other responsibilities/tasks within the LTSS domain or outside of it as requested by your manager.Requirements for the Role:
        • You've worked as an LTSC, or have worked closely with them
        • You understand the various LTS services and their connection to assessments
        • You enjoy working as part of a team and enjoy being a team player who jumps in wherever necessary, and as a self starter who can work independently
        • You are adaptable to a fast-paced environment, organized in your work approach, and able to handle multiple tasks simultaneously.
        • You are comfortable working with new computer applications, electronic medical records, and aimed at providing an innovative experience for our members and our care teams
        • You have excellent communication skills, including the ability to effectively interact with all members of the care team, and with members of the public
        • You are compassionate, open-minded, and non-judgmental
        • You have the ability to think quickly and solve problems in the momentNice to Have, But Not Required:
          • Existing relationships with Massachusetts ASAPs/ILCs
          • Associate's or Bachelor's degree
          • Experience in a primary care setting
