Job Description:

The Admissions Coordinator helps manage the facility's census through maintaining relationships with hospitals and insurers. In addition, to carry out the admissions process by working with new admissions and/or their representatives. Prior experience in a Skilled Nursing setting is required.Beaumont Rehabilitation and Skilled Nursing Centers take an interdisciplinary approach to comprehensive restorative and medical care. Rehabilitation, post-surgical, short- and long-term care, skilled nursing and complex medical management are all provided in our warm, patient-centered environment.COVID-19 Considerations: In compliance with DPH regulations, all staff are required to be fully vaccinated and boosted against COVID-19Responsibilities:Onsite coordination of referrals and screens of potential admissionsMeet with the Director of Nursing (or Assistant DON), Director of Admissions, Charge Nurse (if needed), Rehab. Therapist (if needed) to authorize acceptance of patient.Coordinate the admission with the appropriate facility personnel. Verify insurance coverage when in question. Obtain HMO authorization and levels of coverage prior to admission.Obtain physician coverage for resident prior to admission Meet with resident and/or representative to explain the admission process, obtain the necessary signatures, and distribute pertinent information to the appropriate personnel.Communicate to appropriate staff any changes or updates in pertinent resident information.Arrange for the preparation of the Admissions Packets. Update material included in the Admissions Packets as needed.Arrange for the faxing of information to physicians, pharmacy, and the laboratory.Give tours of the facility and take inquiries from prospective residents and/or family members. Attend family meetings at Whitney Place and the Willows when resident requires placement. Maintain Health Drive authorization notebook. Coordinate eye care appointments prior to and the day of the examinations for all eye care providers. Maintain Podiatry authorization notebook. Maintain inquiry notebook.
