Description$3000 Sign On Bonus BILINGUAL PREFERRED (SPANISH) 6% ONE TIME INCREASE FOR BILINGUAL CANDIDATES Eliot's MSPCC Prevention Program in Worcester is seeking an experienced candidate for an Intake and Outreach Coordinator. This position will develop effective relationships with staff and clients through the utilization of interviewing skills, education, role modeling, and the provision of supportive, concrete services. MSPCCs Healthy Families is a nationally accredited with evidence-based, home visiting, parenting education programs that support young, first-time parents to create stable, nurturing environments for their children. HF is focused on building & promoting protective factors in families. Efforts include supporting positive, effective parenting skills, fostering health, growth & development in infancy & early childhood, promoting educational attainment, job & life skills, reducing repeat pregnancies & promoting parental health & wellness.Responsibilities:

• Determine potential eligibility for programs following intake policies and procedures.
• Responsible for coordinating referrals from multiple programs and agencies in catchment area and general follow up and follow through with those referred.
• Model for staff how to conduct assessment, and develop and implement appropriate service plan utilizing appropriate community resources.
• Attend community meetings and staff meetings as well as any mandated trainings.
• Solicit referrals for all Mass Home Visiting Initiative programs.
• Respond to and follow through with all Mass Home Visiting Initiative referrals.
• Develop effective relationships with staff and clients through utilization of interviewing skills, education, role modeling, and the provision of supportive, concrete services.
• Educate families as to the local available resources.
• Assist with the development of marketing materials.
• Maintain up to date knowledge of the communitys System of Care and the resources that are available to families.
