General Summary:
Responsible for facilitating and coordinating the Key Elements of
the Stroke Certification process, including development and review
of protocols, initial and ongoing training, performance improvement
and community outreach. This will be achieved by collaborating with
the Emergency Department, ED Director, EMS, hospital department
directors and staff. This position may be held in conjunction with
other roles within the hospital.
Requirements:
The ideal candidate will have Program Coordinator/Leadership
experience.
5 years' Clinical experience to include telemetry.
Familiarity with the Neuroscience patient population and Stroke
evidenced based medicine requirements. Excellent customer service
skills. Ability to work independently & be self-directed.
Ability to coordinate & work with Teams at varying levels across
the organization. Proficient in a variety of software and computer
programs to include but not limited to Cerner, Outlook, Excel and
PowerPoint. Knowledge of Process Improvement/Quality management
methodologies to include statistical analysis.
Proficiency in ASA, AHA, GWTG Stroke data collection, data
definitions and abstraction guidelines. Proven superb
organizational, communication and presentation skills.
Knowledge of regulatory compliance & experience in facilitating and
coordinating the Key Elements of the Stroke Certification process,
including development and review of protocols, initial and ongoing
training, performance improvement and community outreach.
- Serves as the key contact for Stroke cases and ensures that the
patient population served has a safe, high quality, efficient
delivery of care throughout the continuum from EMS to post
discharge through compliance with TJC elements of performance and
adherence to ASA/ AHA clinical practice guidelines. Ensure
Neurologists and Mid-levels are entering Stroke Order Sets based on
ASA/AHA Clinical practice guidelines.
- Directs and oversees the Stroke program in collaboration with
the Stroke Medical Director. Facilitates achieving and maintaining
Joint Commission certification as an Advanced Primary Stroke Center
via regulatory standard compliance. Establishes, coordinates and
facilitates plan to maintain Advanced Primary Stroke Certification
in collaboration with Stroke Medical Director. Responsible for
Intracycle data input, preps team and coordinates Intra-cycle call.
Coordinates and directs data pull for on- Site certification Visit,
conducts Joint Commission presentation and accompanies reviewer
during on-site visit.
- Serves as the primary point person for Neurologists with a goal
to continue to grow volume and identify ways to support physicians
in gaining cases.
- Collaborates with the ED Medical Director, Stroke Medical
Director, EMS, Nursing Directors, Radiology, Laboratory and other
hospital departments to develop and implement patient care pathways
based on current research/standards of care. Collaborates with
medical and hospital staff to provide multidisciplinary care to
stroke patients.
- Ensures personnel providing care in the Stroke/Neuro units
obtain and maintain required competencies in collaboration with
Education Department.
- Develops and implement process improvement initiatives for
Stroke Certification in collaboration with ED Director, ED Manager,
Stroke Team and the Quality Management Director.
- Performs daily concurrent and retrospective review of patients
treated with Stroke Clinical Pathways for appropriateness, response
times, performance and outcomes as applicable in collaboration with
members of the interdisciplinary patient care team.
- Analyzes data and prepares reports on clinical care reviews,
reviewing with ED Director, ED Assistant Director, Stroke Medical
Director and appropriate committees as scheduled. Reports data to
STEMI Team/Stroke Team.
- Plans, develops, initiates and evaluates yearly Stroke
performance improvement plan. Provides performance improvement
analysis and collaborates with Physicians and staff to implement
process improvement initiatives.
- Collects and analyzes stroke program patient data to assist
with documentation, clinical care, and improved outcomes.
Responsible for input & maintenance of Stroke Log including DTN,
turnaround times, CT/MRI results. Tracks, trends and shares results
with Neurologists, ED physicians, Radiology, Laboratory and ED to
determine cause for variance. Assists Quality Department as
required in tracking and trending, maintenance of related
databases, i.e. AHA Get with the Guidelines, Joint Commission Core
Measures, and CMS. Presents trends to appropriate
committees.
- Participates in concurrent quality improvement program for
stroke conditions with EMS personnel by review of patient level
data.
- Promote education related to Stroke and cardiac conditions to
identified units and other hospital personnel.
- Support and collaborate with Hospital/Education and Marketing
to develop community outreach/educational programs. Conduct at
least 2 community education events per year with a goal of
providing community education, promoting our programs and growing
volume.
- Utilize an organized and systematic plan for Stroke Care Core
Measure related data abstraction, aggregation and analysis. Works
collaboratively to identify actionable items with PCI abstractor
who oversees data abstraction, aggregation and analysis.
Consistently utilizes error prevention processes during review and
adapt analysis. Report all actual/possible adverse events which may
be identified during record review in relation to Stroke Reviews.
Work's in collaboration with Patient Safety Officer and DCQI to
assist in facilitating root cause analyses as needed.
- Facilitates the identification and interpretation of Stroke
related information, especially related to practice/compliance
patterns, outcomes and trends, including historical and benchmark
comparisons.
- Utilize the result of research findings in presentation of
information and analyses of data. Disseminate information regarding
clinical practice guidelines or research.
- Complete special studies, focus reviews or investigations and
prepare reports regarding Stroke Certification as
requested.
- Chairs Stroke Team (Neuro PI) Meetings. Ensures timely and
accurate communication of Stroke Certification related information
to all participants as required. Prepare agendas and recording of
minutes of Stroke Team.
- Writes, reviews and updates Stroke policies in collaboration
with Stroke Teams. Develops performance improvement program plans,
policies, protocols, and pathways to support timely and effective
care for Stroke patients. Assures compliance with other regulatory
and accrediting standards, all hospital and department policies and
procedures with regard to patient safety and well-being and patient
privacy.
- Represents the Stroke program in various hospital, community,
and Tenet committees to enhance optimal stroke care
management.
- Support Quality PI initiatives related to other service lines
such as Radiology, Oncology, Behavioral Health or Medicine
- Facilitate PI meetings
- Coordinate and participate in PI projects
- Prepare reports and presentations for committees
- Complete chart reviews and auditing for PI projects
- Promote regulatory compliance across hospital departments.
- Additional Responsibilities:
- Provide advice, support and expertise in strategic planning,
scope of service, equipment specifications, space requirements, and
resource allocations.
- Provide input into and assist in setting priorities for both
capital and operating budgets.
- Other duties as may be assigned.
Qualifications:
Education required: Graduated from a state approved school of
nursing, academic degree in nursing Bachelor's or Master's
degree.
Certifications required: Currently licensed, certified, or
registered to practice profession as required by law, regulation in
state of practice or policy; CPR.
